The EGI VO Registration Form can be found here. Note that you will have to read and become familiar with a number of EGI policy documents before beginning the actual registration process. These are listed here.
Once you have completed all of the entries on the registration form and clicked on the Register this new Vo! button, you will receive confirmation of the registration and a link to the GGUS ticket created to monitor the registration process.
The New VO deployment page has extensive information about new VOs (for prospective VO managers and the sites supporting them): all the information required and the steps that should be taken.
